As a Band 5 Community Staff Nurse, you will join our valued Community Nursing Team based in Gravesend, North Kent. This is a rewarding opportunity to deliver care where it matters most, in patients' own homes and local communities. You will support individuals with a wide range of healthcare needs, helping them to manage long‑term conditions, recover from illness and maintain their independence while avoiding unnecessary hospital admissions.

The service operates seven days a week between 8:00am and 8:00pm.
Main Responsibilities:
- As a Community Staff Nurse, you will assess, plan, deliver and evaluate nursing care for patients within their own homes and community settings.
- Working closely with Senior Sisters, GPs, therapists and wider multidisciplinary colleagues, you will help ensure patients receive safe, effective and compassionate care tailored to their individual needs.
- You will manage and prioritise your own workload, responding to changing clinical needs and ensuring patients and carers remain fully involved in decisions about their care.
- You will also play an active role in health promotion, supporting people to improve their wellbeing and maintain their independence wherever possible.
- Alongside your clinical responsibilities, you will contribute to service improvement initiatives, clinical audits and the ongoing development of community nursing services, helping us continue to deliver high quality care across Gravesend.
