Working with your community nursing team, you’ll help deliver safe and person‑centred care that supports people to stay well at home. You’ll be part of an integrated service that provides high‑quality nursing care to adults with a wide range of needs. Your work will help people manage their health, stay independent and feel supported in their own homes and residential settings. You will join a friendly and committed team who work closely with colleagues across the Integrated Nursing and Conditions services. We provide a supportive environment where you can grow your clinical skills and contribute to continuous improvement.

What we’re looking for:

  • A registered nurse with current NMC registration
  • Good assessment, communication and clinical skills
  • Ability to work well in a team and build positive relationships
  • An interest in community nursing; you may be newly qualified or experienced

What you’ll do:

  • Carry out holistic assessments and develop clear, person‑centred care plans
  • Deliver safe and effective nursing care in line with each person’s needs

  • Use and update the Electronic Patient Record (EPR) to maintain accurate notes
  • Support people, families and carers to understand and manage health needs
