- 1. Responsible for effective communication with multi- professional team members/line manager and other support staff
- 2. Report to supervisor any patient problems, issues regarding clinical management of patients, from yourself or other community rehab workers
- 3.
Communicate effectively, sensitively and appropriately with patients (and their carers) who may have a variety of medical, psychological and social needs, including those with impaired cognitive function i. e. stroke, deaf patients.
This may involve communicating difficult or distressing information to patients about a change in function or ability.
- 4.
Answers telephone and responds to patients enquiries, having the ability to problem solve and be the first line of enquiry relays information accurately to Therapists when relevant.
- 5. Complete all necessary records, reports and statistics.
- 6. To support with the allocation and scheduling of ICT visits
- 7. Liaise with other agencies that may support patients on discharge including Community Equipment Store
- 8. Key function to sign in and out staff at beginning and end of day showing initiative if problems arise, problem solving and escalation as required by service processes
- CLINICAL
- 1. Initiate assessment, treatment and discharge of patients within Urgent Community Care as delegated by Team leads (as appropriate) without direct supervision.
- 2. To facilitate the safe and timely discharge of patients with non -complex needs from the Department/ assessment including comprehensive risk assessment and management.
- 3. To carry out community visits as appropriate to facilitate safe discharge within agreed protocols and guidelines and appropriate to in-patients/community therapy team
- 4. When competent as a delegated task from a registered health care professional (RHCP) to complete a full set of observations/vital signs
- 5. When competent as a delegated task from a registered health care professional (RHCP)to complete a Restore / Soft signs assessment
- 6. When competent as a delegated task from a registered health care professional to administer medication from a medicine administration record (MAR Chart)
- 7. To use a range of verbal, non verbal and physical skills to plan and deliver physical rehabilitation using evidence based practice.
- 8. Triage patients to other staff in the team and staff that they line manage ( e. g. assistants and technical instructors)
- 9.
To formulate and write a problem list, goals and treatment/discharge plan for each patient according to the department documentation and the unit guidelines.
Document all treatments.
- 10. Prioritisation of own patient list and tasks. Responsible for planning and organisation of own patient caseload.
- 11. Attendance at relevant multi professional team meetings reporting on own behalf as appropriate.
- 12. Complete relevant ongoing documentation/ referrals and assessment paperwork in relation to a patients ongoing care.
- 13. Analysis skills required to trouble shoot where situations are non routine.
- 14. Discuss with supervisor/ team leader any problem patients and if a patient fails to progress.
- 15. Have an active role in supporting daily handover
- CLINICAL GOVERNANCE
- 1. Assists in clinical audit to review the effectiveness and efficiency of the service.
- 2. To contribute to service developments, delivery and evaluation within clinical area.
- 3. Provide the Team Leader with a monthly report as per department guidelines.
- 4. Attend all statutory and mandatory training.
- 5. Attend relevant in service at a department and unit level.
- 6. Keep own workload statistics and collate those for team if relevant.
- 7. Be aware of, and contribute to the teams overall annual plan.
- 8.
Actively participate and be responsible for own staff development and review including achievement of core competencies, appraisal and documentation with support from Senior staff.
Maintain own continuous development portfolio.

- FINANCIAL/ RESOURCES
- 1. To co-ordinate and monitor stock levels of stationary, equipment.

- 1. Day to Day line management of assistants or technical instructors. Undertake objective setting/ appraisals
- 2. Co- ordinate and deliver specific assistant training, reinforcing and signing off core competencies, with support of senior staff.

- 2. Competent in the use of hospital computer system to ensure accurate statistical recording and notes/ discharge letters when relevant.
- HEALTH AND SAFETY
- 1. Comply with all local Health and Safety trust policies.
- 2. Undertake appropriate risk assessment prior to all treatments.
- 3. Ensure safety of patients and staff and report problems to line manager taking any necessary remedial action to avoid accident or injury, complete incident forms.
- 4. Perform safety checks on equipment and aids as per department standards prior to issuance to patients.
- GENERAL
- 1. Ensure confidentiality at all times (as per Caldicott)
- 2. Undertake any other duties as may be required by line manager.
- 3. Comply with all Trust and department policies
- 4. Only treat patients within a defined scope of practice/ competence.
