What You’ll Be Doing

  • Comprehensive Care: You’ll lead systematic nursing assessments that consider the "whole person"—including their culture, environment, and psychology.
  • Collaborative Planning: Working side-by-side with service users, their carers, and a multi-disciplinary team, you’ll co-create person-centered care plans that actually mean something to the individual.
  • Clinical Excellence: You’ll act as a mentor and leader, ensuring our practice stays in line with NICE guidance while delivering hands-on care and evaluation.
  • Service Evolution: From facilitating Depot and Clozapine clinics to driving Quality Improvement (QI) initiatives, you’ll help us constantly find better ways to serve our community.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Supporting and assessing patients to function to their optimal ability in their community environment. Maintaining and promoting a safe environment taking account of infection control and identifying and reporting hazards and risks in a timely manner. May also be required, at times, to work at other Trust units and/or sites. Physically able to carry out all duties required of the care setting, following successful completion of physical interventions training as required.
