As a Care Coordinator, you will have a registered profession and have a caseload of clients which would involve monitoring and assessment of risk and coordinating treatment plans.
Work with other professions within the service including:
- STR workers
- Advanced Clinical Practitioners
- Pharmacist
- Medics
- Psychology
- Individual Placement Support
to ensure the service user is offered a range of support which meets the individuals and families needs.
Offer physical health monitoring to clients under the service.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
