What you can add

  • Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
  • Signposting to appropriate care providers (including primary and tertiary care).
  • Raising concerns to senior staff for support and management.
  • Completingdocumentation and planning care for patients.
  • Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.
  • The role will involve, but not be limited to you performing the above.

What you'll need

  • A minimum of 6 months paid experienceas a Community Psychiatric Nursein an UKhospital environment within the last 3years
  • Comply with NMC Professional Registration and keep up to date with current nursingpractices
  • Full UK driving licence and own vehicle.
  • Depending on the Trust, you may require variable additional certification and skills.
