Oxleas NHS Foundation Trust is seeking a Mental Health Practitioner to provide specialist mental healthcare to offenders as part of the Mental Health In‑Reach Team and wider MDT. The role involves delivering interventions at primary and secondary care levels, including referral management, screening, assessment, care planning and risk assessment. You will work with prisoners across multiple sites within the Kent cluster, supporting continuity of care and contributing to MAPPA and joint agency.
