Participate in multidisciplinary team meetings to provide specialist medicines management advice and support to the ICT MDT.

Provide specialist advice and practical support to patients regarding their medicines, to family members and carers as appropriate, and to other health and social services staff as necessary.

Explore reasons for non-compliance, both intentional and unintentional, and develop strategies and methods to reach concordance, tailored to the individual.

Assess, plan, implement and evaluate skilled medicines management interventions for patients across the community/primary care interface, in co-operation with other members of the ICT/integrated health and social care teams.

Complete documentation to record clinical data, recommendations and outcomes.

Report pharmaceutical errors/ incidents and issues as appropriate using the Trust incident reporting scheme.

Contribute to the investigation of incidents, including attending safeguarding reviews etc.

Contribute to overall patient health and social care through identification of non-medication related issues.

Refer to the appropriate agencies as required.

Educate and support the members of the multi-disciplinary teams with regard to medicines management issues.

Devise and conduct training for others, including healthcare professionals, care staff, managers, members of the workforce development team as necessary, in aspects of domiciliary medicines management relevant to them.

Contribute to the development and direction of the service.

Evaluate the service through audit, surveys and other appropriate means.

Be aware of the risks of lone working and take steps to manage these appropriate to the individual situation.

Be able to respond to situations encountered in the home, including knowing who to call for help if necessary.

Work with local, regional and national bodies and contribute to the development of medicines management training courses ensuring that they include both primary and secondary care aspects of medicines management.

Support any other duties as deemed necessary by the Chief Pharmacist, Pharmacist Team Manager or line manager.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines.

You will need to hold a full UK driving licence* and have use of a vehicle.

You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
