Referral Coordinator Location: Head Office Reports to: Operations Director Salary: Β£32,000Β£38,000 per annum + Performance Bonus

The Role

The Referral Coordinator plays a vital role in driving occupancy across Isabella's Homes by managing referrals from local authorities, NHS partners and commissioners. You'll coordinate assessments, liaise with professionals and ensure referrals progress efficiently from enquiry through to admission.

Key Responsibilities

  • Manage incoming referrals.
  • Build relationships with commissioners and social workers.
  • Coordinate assessments and viewings.
  • Track referral pipeline and occupancy.
  • Prepare placement proposals and costings.
  • Work alongside Operations and Registered Managers.
  • Ensure referral documentation is accurate.
  • Monitor KPIs and reporting.
  • Attend networking events where required.
  • Support mobilisation of new services.

Essential Requirements

  • Experience within supported living, social care or healthcare.
  • Excellent organisation skills.
  • Strong relationship-building abilities.
  • Commercial awareness.
  • Ability to manage multiple priorities.
  • Confident communicator.
