The predicted salary is between 37035 - 45265 Β£ per year.
The post holder will provide specialist learning disability liaison nursing support to adults with learning disabilities and/or autism accessing services at Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust (Doncaster Royal Infirmary, Bassetlaw Hospital, Mexborough Montagu Hospital) to ensure facilitation of open and easy access to the various departments in the acute services.
Manage a defined caseload of adults with learning disabilities and/or autism requiring specialist liaison input within acute hospital setting. Identify patients requiring reasonable adjustments and support DBTH to ensure relevant personalised care plans. Support admission planning, inpatient care and discharge processes. Act as an advocate for people with learning disabilities accessing healthcare services.
Provide specialist advice and support regarding:

- Mental Capacity Act
- Consent to treatment
- Health inequalities
Participate in multidisciplinary meetings and case reviews. Facilitate effective communication between acute staff, carers, community teams and other agencies. Contribute to safeguarding processes in accordance with Trust policies.
