An exciting opportunity has arisen for an experienced and motivated leader to join our Community Nursing Service as a Team Lead. As part of our evolving Neighbourhood Team model, you will play a pivotal role in leading multidisciplinary community teams to deliver high-quality, safe and effective care to patients in their own homes and local communities. You will provide visible leadership, operational oversight and professional support to a diverse workforce, ensuring services are responsive, person-centred and aligned to organisational priorities.

The Team Lead will provide visible operational and professional leadership to nursing teams working within community settings, supporting the delivery of safe, effective and person-centred care.
Main responsibilities include:
- Providing visible leadership and support to staff, creating a positive team culture that promotes wellbeing, engagement and continuous improvement.
- Managing workforce performance, recruitment, attendance, supervision and professional development.
- Overseeing the day-to-day delivery of services, ensuring resources are effectively deployed to meet patient and service needs.
- Working collaboratively with clinical, operational and multidisciplinary colleagues to coordinate care and improve outcomes for patients.
- Monitoring quality, performance and patient experience, taking action to address risks, concerns and opportunities for improvement.
- Leading and supporting service developments, quality improvement initiatives and organisational change programmes.
- Ensuring compliance with professional, organisational and regulatory standards, maintaining a strong focus on patient safety and governance.
