Provision of Clinical Pharmacy service to allocated teams. Provision of high quality patient focused pharmaceutical care. Provision of professional responsibilities of a pharmacist supervising the dispensing of prescriptions at Fieldhead Pharmacy whilst fulfilling the dispensary commitment. Participation in the Saturday, Bank Holiday and On-call Rotas.

In conjunction with the pharmacy team, develop and implement safe and effective policies and procedures in line with New Ways of Working. To provide management to allocated staff including appraisal, development and clinical supervision. To be involved in the development of new services as allocated by pharmacy management team. Develop and participate in Trust-wide medicines management audits and attend allocated governance groups to represent pharmacy.

KEY RESULT AREAS:

  • To be responsible for the clinical pharmacy and prescription monitoring at allocated clinical teams.
  • Be a specialist medication referral point for these teams.
  • Performance indicators: Medicines reconciliation targets for inpatient areas.
  • Ensure all service users have access to clinical pharmacy services.
  • Screening for all service users admitted to intensive home based treatment services.
  • Develop and contribute to patient focused services to allocated teams, including service user groups.
  • Provide appropriate and timely verbal and written information on all aspects of medication usage to service users, carers and healthcare professionals within SWYPFT.
  • Participate in in-house training for pharmacy staff and healthcare professionals, both attending and delivering training.
  • Participate in the training of pre-registration pharmacy graduates and mental health pharmacy diploma students.
  • Participate in the development of the medicines management clinical audit programme in particular the POMH audits and ensure roll out to allocated units.
  • Participate in research and development as required for the development of the clinical pharmacy services.
  • To cover for other senior pharmacists in their absence.
  • If required due to sickness or other absence to work in other localities of the organisation.
  • To carry out CPD in line with the GPhC requirements.
  • To maintain and demonstrate a good clinical pharmacist knowledge.
  • To work within the policies and procedures of the South West Yorkshire Partnership NHS Foundation Trust and also to ensure compliance with the pharmacy policies and procedures from other organisations when working in partnership.
  • To participate in the Trusts Appraisal system.
  • Following qualification as an independent non-medical prescriber, prescribe medication as an independent prescriber within their own sphere of competence and the bounds of their approved prescribing status in accordance with the Trust non-medical prescribing policy.
  • Promote patient choice and understanding when communicating assessment outcomes or prescribing medicines, including provision of high quality information both written and verbal to support this.
  • Be responsible for ensuring the security and safe handling of FP10 prescription forms and to report any loss or theft as per Trust guidelines.

Know the action to be taken in the event of a fire and must undertake fire training annually; receive supervision in line with the Trusts Supervision Policy and an annual Appraisal in line with the Trusts Appraisal Policy, during which mandatory, role specific and personal development needs should be identified and agreed; understand their responsibilities under the Trusts Equal Opportunities in Employment Policy and ensure that they adhere to the provisions of the policy; recognise, respect and support the equality diversity of staff, colleagues, service users, carers and the public. Contribute to a working environment which promotes and responds positively to difference and diversity; ensure they carry out their duty to safeguard and promote the welfare of children and young people under the age of 18 years, as issued under Section 11 of The Children Act 2004, by being familiar with and adhering to Trust safeguarding policies and participating in relevant training; comply with their professional responsibilities to develop their practice and deliver care through a Clinical Governance framework (i.e. CPD, Audit, Supervision); demonstrate, through practice and practical understanding, the importance of the continual development of individual, team and service wide quality improvement; abide by relevant codes of professional practice, with the organisation taking action when codes of conduct are breached; work flexibly to meet the needs of the service/organisation, whilst working within a culture of progressive employment practices and commitment to the Investors in People (IiP) Standards; adhere to the Trusts smoke free policies, which prohibits smoking anywhere on Trust grounds; being socially responsible by complying with measures that support the Trust in reducing or offsetting our environmental impact; Identify and report risks, hazards, incidents, accidents and near misses promptly; In addition to the Trusts own responsibilities under the Health and Social Care Act 2008, Code of Practice on the prevention and control of infections and related guidance, for your safety, ALL staff (and contractors) are responsible for ensuring their work adheres to this Code in the delivery of safe patient care.
