The post holder will provide specialist dietetic assessment, treatment and nutritional support for adults admitted to inpatient wards and at least two sessions per week in the community. They will independently manage a complex caseload, undertake comprehensive nutritional assessments, develop and review individualised care plans, and deliver evidence-based interventions to improve patients' physical health and nutritional outcomes.

Working collaboratively within the multidisciplinary team, the Dietitian will support nutrition screening, weight management, oral nutritional support, discharge planning, and referrals to specialist services where required. The post holder will also deliver staff training, contribute to quality improvement and service development projects, supervise junior staff and students, and ensure accurate clinical documentation and audit in line with professional and Trust standards.

The post will work collaboratively with other AHPs to provide a community physical health clinic as part of our neighbourhood transformation offer. Provide Dietetic input for adults diagnosed with mental ill health within C&H services. Independently manage a caseload using clinical reasoning to support Dietetic assessments and interventions, with peer-support, and supervision from the professional development lead dietitian.

Use a person-centred approach and current evidence base to assess, plan, implement, evaluate and document Dietetic interventions. Prioritise a caseload of people requiring nutrition support by working closely with the MDT, attending team meetings, and ensuring appropriate review and monitoring.

Support the delivery and audit of Nutrition Screening and Care Planning Training for staff. Work with staff to promote cost-effective oral nutritional supplement prescriptions in line with local formularies. Collaborate with the local Food Committee, service leads and service users on Quality Improvement and Service Development projects to improve nutrition outcomes.

Support the development of Dietetics service pathways. Support with effective onward referrals and discharge plans to appropriate services including eating disorder, local weight management, enteral feeding and other physical health services. Participate in ward/unit based health promotion and quality improvement activities with other professionals as capacity allows.
