Elysium Healthcare is seeking a Social Worker for The Woodmill, Cullompton, to support step-down recovery and vulnerable adults.

You will manage caseloads, conduct social care assessments, and contribute to CPA/MH reports during discharge planning.

Based in Cullompton, you will collaborate with the MDT, liaise with safeguarding and Local Authority, and help access welfare rights and housing.
