Central Cheshire Integrated Care Partnership (CCICP) Tissue Viability Team has an exciting opportunity for an enthusiastic, dynamic, highly motivated and forward-thinking registered nurse to join our team who have a passion for wound care. We are looking to recruit permanent staff member/s.
You will need to have had experience of caring for patients with a wide range of wounds/significant, post-registration experience within wound care, be proactive in the prevention of skin breakdown and have the experience/or confidence to be involved in both formal/informal education delivery.
The role includes acting as a specialist nurse in all aspects of Tissue Viability & you must be willing to gain a qualification in Tissue Viability as the role develops. The role will include undertaking a holistic assessment of patients with complex wounds considering physical, psychological, emotional, & social needs, implementing plans of care & lifestyle changes, to enhance their quality of life.
The applicant should have excellent communication skills to deliver health promotion strategies to patients & other health care professionals. This post will allow you to expand on your skills in delivery of training and assessment to support all patients in the community/acute setting with regards to excellent Tissue Viability management.
The applicant will be expected to work across South Cheshire and Vale Royal (CCICP), involving the five care communities, acute settings, nursing homes, GP practices and specialist schools in maintaining high standards of patient care in reducing pressure ulcer incidence and managing complex and challenging wounds. The role requires you to be flexible. You should have excellent time management skills and be able to prioritise your workload daily.
The successful applicant will support the Clinical Lead in implementing new ways of working. They will be working within the acute setting/community teams and therefore need to be able to have the ability to work autonomously and as part of a team assessing, triaging, planning, and implementing specialist wound care advice and management.
The applicant should have good knowledge of adult and children's safeguarding procedures and protocols. The individual should have sound knowledge of NICE guidance, best practice and evidence-based practice related to Tissue Viability. Good knowledge of our current pathways would be of an advantage.
