We have an exciting opportunity for an experienced nurse/physiotherapist with an enthusiastic interest in Adult Respiratory Medicine.

We are a seven day service consisting of a specialist team of Nurses and Physiotherapists who work primarily to meet the needs of patients with long-term respiratory conditions.

The role will include conducting face to face assessment and reviews of both long-term respiratory conditions and acutely unwell people within their own homes, hospital and a clinical environment.

We also provide the home oxygen assessment and review service for all adult patient regardless of diagnosis.

Post registration experience in a relevant field for a minimum of 3 years is required.

You will act as an autonomous practitioner within the specialist team providing both community care and in-reach into the acute trust. This includes a mixture of face to face and telephone assessment of people with Respiratory or long-term conditions. You will be flexible to meet the needs of a reactive service and should be able to prioritise your workload based on patient/service need.

  • JOB PURPOSE
  • To provide the specialist practitioner function in the delivery of specialist care to patients who have been referred to the Integrated Respiratory team.
  • To provide specialist input into the assessment, monitoring and evaluation of chronic respiratory patients within acute, outpatient and community settings and lead in the provision and running of Oxygen assessment clinics.
  • To be involved in developing and delivering high quality patient centred care and working with the associated clinical team to ensure this is incorporated within the Trust objectives.
  • SERVICE DELIVERY
  • Works autonomously to manage his/her own caseload of patients whilst working as part of a multidisciplinary team.
  • Enables patient choice and involvement and initiates appropriate action.
  • Co-ordinates patient care pathway and liaises across organisational boundaries.
  • Plans specialised programmes of care and provides specialised advice to patients, relatives and carers.
  • Ensures each patient is placed in the correct treatment pathway at the appropriate time and necessary investigations and treatments are organised.
  • Recognised and responds appropriately to urgent/emergency situations.
  • Provides clinical advice and support to healthcare professionals within and across organisational boundaries.
  • Provides clinical advice in accordance with current evidence based practice.
  • Participates in nurse/practitioner -led clinics, and multidisciplinary team meetings.
  • Participates in case conferences in relation to future management or discharge arrangements.
  • Ensures patients and their families are given appropriate oral and written information regarding planned care.
  • Maintains accurate patient records and ensures all relevant information is documented in the patient's medical and nursing records.
  • Working towards becoming an independent non-medical prescriber (adhering to Trust policy) with the ability to initiate and adjust medication within your field of expertise.
  • Participates in and undertakes clinical audit and contributes to development of knowledge within specialist field of practice.
  • To participate in undertaking research in relation to specialist area of practice
  • Undertakes evidence based practice and ensures dissemination of relevant research and best practice
  • Supports and advises junior staff who may wish to undertake audit and research in specialist field.
