Are you an experienced and motivated Physiotherapist looking to take the next step in your career?

We are offering an exciting 12-month fixed term/secondment opportunity within the Coventry Integrated Neighbourhood Teams– a pioneering service that brings together health and social care professionals to deliver seamless, person-centred care.

As a Physiotherapist, you will manage a varied caseload, providing specialist assessments and evidence-based interventions to patients with a wide range of health conditions across community settings, including patients' homes and short-term placements.

You will be part of an innovative team supportingreablement, hospital discharge, and admission avoidanceacross urgent and emergency care pathways.

Working collaboratively with multidisciplinary colleagues, you will help shape and influence the delivery of integrated care for the people of Coventry.

Secondment will be considered for internal candidates – If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Using your clinical expertise and sound professional judgement, you will work collaboratively with colleagues, patients, carers, social care partners, and third-sector organisations to support individuals to achieve their goals, maximise independence, and remain at home wherever possible.

You will undertake holistic assessments, develop tailored care and therapy plans, and deliver interventions that address patients' physical, cognitive, psychosocial, behavioural, sensory, and environmental needs.

The successful candidate will possess strong knowledge across a variety of clinical areas, including neurological, orthopaedic, musculoskeletal, respiratory, falls, and care of older people.

You will also contribute to service improvement initiatives, support effective information management and data quality, and work alongside team leaders and managers to enhance the delivery of integrated community services across Coventry.

  • To provide specialist physiotherapy assessment, treatment and review of patients with a wide variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls, to reduce unnecessary hospital admissions, contribute to effective admission avoidance and support patients with complex health care needs.
  • This will be for patients predominantly in their place of residence, but will include short term placements and other settings.
  • To provide advice and guidance to others, patients, carers and staff working in health and social care.
  • To support professional development of the staff within Coventry Integrated Neighbourhood teams and other services as appropriate.
  • To support and contribute toward service development and meeting Key Performance Indicators.
  • To participate in 7 day working rotas to ensure delivery of key services.

The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders.

This pilot aims to explore team structures and assess the impact on patient experience.

While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.

If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model.

However, any formal changes will be preceded by appropriate consultation in line with organisational policy.
