Paediatric Diabetes Lead Specialist Nurse | University Hospitals Coventry and Warwickshire NHS Trust

An exciting opportunity to provide high standard care to children and families with Diabetes.

You will work collaboratively with, and lead a team of Paediatric Diabetes Specialist Nurses.

You will work alongside an experienced paediatric consultant with specialist expertise in Diabetes.

An extended multidisciplinary team will also be on hand to offer support.

The post holder will work across the UHCW and Rugby St Cross site, whilst occasionally needing to attend meetings within community settings.

The post holder will be involved in developing and delivering expert patient-centred care based on local and national guidance.

Care delivered will include patient assessment, planning and implementing of specialist care, provision of advice/support and education in collaboration with the wider MDT including child protection/safeguarding teams.

The post holder will be involved in audit, assist in leading a small team, participate in network meetings and provide education to medical and nursing staff within the unit in order to improve quality and enhance care delivery.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

The post holder is an expert clinical practitioner in paediatric diabetes, working autonomously as a key member of the multidisciplinary team (MDT).

They will play a pivotal role in the delivery and development of paediatric diabetes clinical pathways, ensuring the provision of high-quality, patient-centred care that achieves excellent clinical outcomes.

The post holder will fulfil the integrated roles of clinician, educator, researcher, and leader, providing expert care, professional leadership, and strategic direction to support the delivery of specialist paediatric diabetes services.

  • Coordinate the care of CYP with diabetes, providing specialist assessment, treatment and ongoing management across the care pathway.
  • Manage an autonomous caseload, delivering child-focused and family-centred care in hospital, community, educational and home settings.
  • Lead nurse-led clinics, undertake comprehensive assessments, formulate management plans and interpret specialist investigations.
  • Independently adjust insulin regimens and support advanced diabetes therapies, including insulin pump management, in line with Trust policies and competencies.
  • Provide specialist advice and support to CYP, families, carers and healthcare professionals in the management of routine and complex diabetes needs.
  • Act as a patient advocate, supporting informed decision-making, self-management and navigation of healthcare services.
  • Deliver timely specialist support during periods of acute illness, crisis or increased clinical need.
  • Work collaboratively with acute, community, network and regional services to ensure seamless, coordinated care.
  • Promote health education, wellbeing and self-management, empowering CYP and families to manage diabetes effectively.
  • Support the delivery of effective transition services between paediatric and adult diabetes teams.
  • Maintain accurate and timely clinical documentation, including use of relevant electronic systems and the Diamond database.
  • Support National Paediatric Diabetes Audit (NPDA) submissions and promote high standards of data quality.
  • Promote and safeguard the welfare of children and young people, participating in safeguarding activities and meetings as required.
  • Adhere to infection prevention and control policies and act as a positive clinical role model.
