Job Summary

We're currently supporting Cheshire and Merseyside ICB in recruiting experienced CHC Nurse Assessors to join its Individual Assessment and Commissioning Team.

This role would suit nurses with a background in Continuing Healthcare (CHC), community nursing, mental health, learning disabilities, complex care or clinical case management.

You'll assess care needs, manage a caseload and work with NHS services, local authorities and care providers to help individuals access the right care and support.

Hybrid working with approximately one office day per week in Merseyside and full-time Monday to Friday hours make this an excellent opportunity for nurses looking to use their clinical expertise in a more autonomous role.

Who We're Looking For

You're an experienced nurse who enjoys assessment-based work, managing a caseload and working collaboratively with others to support positive outcomes.

You'll be confident making informed clinical decisions and communicating with healthcare professionals, service users and families.

Key Responsibilities

  • Complete Continuing Healthcare (CHC) assessments and reviews.
  • Manage a caseload of individuals with complex health needs.
  • Review and coordinate packages of care.
  • Support NHS-funded Nursing Care and Fast Track pathways.
  • Work closely with NHS services, local authorities and care providers.
  • Maintain accurate clinical records and documentation.

Qualifications

  • Current NMC registration as an RGN, RMN or RNLD.
  • Right to Work in the UK.
  • Experience in Continuing Healthcare (CHC), community nursing, mental health, learning disabilities, complex care or clinical case management.
  • Strong assessment and clinical decision-making skills.
  • Excellent communication and relationship-building skills.
  • Ability to work independently and manage competing priorities.
  • Knowledge of NHS-funded Nursing Care and Fast Track processes.
  • Experience working across health and social care services.
  • Knowledge of the Mental Health Act, Mental Capacity Act or Section 117 pathways.
  • Experience reviewing or commissioning care packages.
