Everliving Services is looking for a compassionate, reliable and flexible Community Support Worker to provide high-quality, person-centred support in clients’ homes and within the community.
You will help clients live safely and independently while promoting their dignity, wellbeing, rights and personal choices.
Key responsibilities
Provide care and support according to each client’s individual care plan.
Support clients with appointments, shopping, social activities and planned outings.
Drive clients safely when required as part of their support.
Assist with daily living tasks and therapeutic activities.
Encourage independence, confidence and community participation.
Observe clients’ health, wellbeing and behaviour, reporting concerns promptly.
Maintain accurate care records, diaries and other required documentation.
Build positive relationships with clients, families, colleagues and professionals.
Respect each client’s culture, values, religion, communication needs and choices.
Follow safeguarding, confidentiality, medication, infection control, health and safety, and risk-management procedures.
Attend required training, supervision and staff meetings.
Work flexibly according to the needs of clients and the service.
