Community Learning Disability Nurse | East London NHS Foundation Trust

The Community Learning Disability Service provides integrated specialist health and social care for adults with learning disabilities in Tower Hamlets.

The service also manages a specialist day activity project called CREATE.

  • **Key Responsibilities:**
  • i. Utilize specialist nursing skills to work with individuals with learning disabilities, focusing on the maintenance, promotion, and improvement of their health.
  • ii.

Support people with learning disabilities in reaching their full potential by minimizing the impact of their impairments, which could hinder their social inclusion.

This will be achieved through a person-centered approach that promotes choice, independence, and inclusion.

  • iii. Facilitate access to healthcare services by collaborating and partnering with primary and secondary health services, as well as other relevant organizations.
  • iv. Contribute to the development of local health services by shaping protocols and policies to promote an inclusive service for individuals with learning disabilities.
  • v.

Actively participate in planning and developing shared care with primary and secondary health services, including roles such as GP Link, Nurse-Led Epilepsy Clinic, and Sexual Relationship & Health Promotion.

  • vi. Conduct assessments as required by the service.
  • The post holder will be a qualified learning disability nurse but work with clinical supervision provided by a specialist community practitioner in learning disability to undertake:
  • Promote health facilitation to reduce health inequalities for the learning disability population in tower hamlets

i. To use specialist nursing skills in working with people with learning disabilities to maintain, promote and improve health.

To support people with learning disabilities to develop their full potential by reducing the impact of their impairment that could limit their social inclusion in a person-centred approach to promote choice, independence and inclusion, iii. To facilitate the access to health care services through collaborative and partnership working with primary and secondary health, or other relevant services.

iv. To contribute to shaping local health services (e. g., development of local protocols and policies), promoting an inclusive service for people with learning disabilities.

Actively participate in the planning and development of shared care with primary and secondary health services, e. g., GP Link Role, Nurse Led Epilepsy Clinic, and Sexual Relationship & Health Promotion etc.

vi. This role involves undertaking a number of assessments as per service requirement vii.

You will have substantial knowledge of the health and social needs of individuals with learning disabilities: awareness of current legislation and best practice in the field of learning disability nursing.
