We are seeking a compassionate and motivated Community Assistant Practitioner to support patients in their own homes, care homes and community settings. Working as part of an integrated multidisciplinary team, you will deliver rehabilitation, long-term condition and palliative care support, helping people maximise their independence and avoid unnecessary hospital admissions.
Applicants should hold an NVQ Level 3 and Foundation Degree (or equivalent experience), have experience in health or social care, excellent communication skills, and a full UK driving licence with access to a vehicle. This is a rewarding opportunity to make a real difference in your local community.
To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, Community Hospitals, and clinics, by assessing, planning, implementing and evaluating packages of care. To work within a reablement philosophy, using strengths-based assessments and actively promoting independence for the patients.
- To contribute to the delivery of care packages for patients with long term conditions, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
- To work within the integrated team to facilitate early discharge from hospital.
- To work within the integrated team to prevent unnecessary admission to hospital.
- To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
