We have an exciting opportunity for a Part-Time Care Coordinator to join our team!
The Role will be
Office Base: (Colchester)
Working Hours: 9am to 5pm (Monday, Tuesday and Friday (Inc on-call) plus On-Call 3 Weekends per month
Job Type: Permanent Part-time (potential full time)
Responsibilities
- Act as the primary point of contact for clients regarding staffing requirements and operational matters.
- Build and maintain strong professional relationships with care homes, hospitals, supported living services, nursing homes, residential homes, and domiciliary care providers.
- Respond promptly to staffing requests and coordinate suitable staff to meet client needs.
- Conduct regular communication with clients to ensure satisfaction and identify opportunities to improve service delivery.
- Resolve client concerns, staffing issues, and complaints professionally and efficiently.
- Arrange and attend client meetings and site visits where required.
- Ensure contractual staffing requirements and service level expectations are consistently met.
- Ensure continuity of care by arranging suitable cover for urgent and unplanned staffing gaps.
- Where all reasonable efforts to source appropriate agency staff have been exhausted, undertake client shifts personally to maintain safe staffing levels and meet contractual obligations.
- Collaborate in multi-disciplinary assessments to understand Service User needs and develop personalised care plans, utilizing the Care System.
- Identify special care requirements based on specific clinical problems and provide ongoing monitoring and review.
- Liaise with external organisations such as Care Homes and other nursing professionals to develop comprehensive care plans for Service Users.
- Regularly report to the Operational Manager on activity levels and outcomes regarding staffs, client and Service Users.
- Engage in staff development, training, and performance appraisals to maintain high-quality care services.
- Adhere to organisational policies and comply with Health & Safety legislation and statutory regulations.
- Maintain good record-keeping practices and provide written reports on complaints, incidents, and untoward events, following established policies.
- Stay up to date with care industry developments, Contracting Authority philosophies, and policies, sharing relevant information with the team and modifying policies as needed.
- Participate in Staff, Team, and Quality Management Review Meetings as directed by the Director.
- Undertake on-call duties as required to ensure 24/7 service provision.
- Coordinate and allocate agency staff to client shifts based on qualifications, experience, availability, compliance status, and client preferences.
- Prepare, manage, and update daily, weekly, and monthly rotas across multiple client services.
- Monitor staff and client communications, manage rotas efficiently, and respond to client queries and complaints.
- Manage last-minute cancellations, sickness, annual leave, and emergency staffing requirements.
- Support the recruitment and selection of new agency staff.
- You may be required to conduct interviews and register care workers and support workers.
