The Patient Flow Coordinator plays a key role in supporting the Nurse in Charge to ensure safe, timely and efficient patient flow through the Medical Assessment Unit. The role focuses on coordinating admissions, transfers and discharges, reducing delays, and ensuring patients move through the system in a timely and well-organised way.

Working closely with the wider MDT, bed managers and ward teams, you will provide real-time updates, elevate delays, and help drive timely decision-making. This includes supporting discharge processes such as TTO requests, urgent summaries, and following up outstanding actions to avoid unnecessary delays. You will maintain accurate patient tracking systems and records, ensuring information is up to date, confidential, and compliant with Trust policy.

Strong prioritisation and communication skills are essential, as you will need to manage competing demands, elevate concerns appropriately, and support safe patient flow during busy periods. The role also involves supporting basic operational tasks such as preparing bed spaces and assisting with patient movement when required. You will consistently promote dignity, compassion, and excellent communication with patients, relatives, and staff, while contributing to service improvement and maintaining a calm, organised approach in a fast-paced environment.
