An exciting opportunity has arisen for a motivated and compassionate Band 5 Community Mental Health Practitioner to join Chorley Community Mental Health Team. We are a friendly, supportive and forward-thinking multidisciplinary team committed to delivering high-quality, person-centred care. We work collaboratively with service users, families, carers and partner organisations to help people manage their mental health needs, build on their strengths and progress towards meaningful recovery.
Chorley CMHT values professional development, innovation and teamwork. You will receive regular clinical and managerial supervision and will be supported to develop your knowledge, confidence and community mental health practice.
As a Community Mental Health Practitioner, you will:
- Undertake comprehensive, person-centred assessments.
- Develop, implement and review collaborative care and safety plans.
- Manage a defined caseload of people experiencing a range of complex mental health needs.
- Complete effective risk assessments, formulations and safety-management planning.
- Deliver evidence-based interventions that promote recovery, independence and wellbeing.
- Work closely with service users, families and carers.
- Monitor physical health and prescribed treatments, including psychotropic medication.
- Contribute to multidisciplinary meetings, clinical reviews and service-development initiatives.
- Work collaboratively with primary care, social care, inpatient services, urgent-care teams and voluntary, community, faith and social enterprise organisations.
- Maintain clear, timely and accurate clinical records.
