The Enhance Clinical Practitioner (ECP) role is based across community bases within Swindon or Wiltshire Hospital at Home service.

You will deliver high-quality, acute-level care to patients in their own homes, supporting a step-up/step-down model and helping prevent unnecessary hospital admissions.

You will work with a forward-thinking MDT in a dynamic, patient-centered environment.
