Are you a compassionate and experienced Registered Nurse looking to make a real difference to people with complex health needs?

We are seeking a motivated CHC Clinical Assesorto join our Continuing Healthcare (CHC) and Complex Care Team.

In this rewarding role, you will support adults with complex and long-term healthcare needs by undertaking assessments, coordinating care packages, and ensuring high-quality, person-centred care.

Working across health and social care systems, you will advocate for service users and their families, helping them navigate the CHC process and access the right support at the right time.

You will work closely with multidisciplinary teams, care providers, local authorities and partner organisations to deliver safe, effective and integrated care.

The role also involves safeguarding, quality monitoring, risk management and contributing to service improvement initiatives.

We welcome applications from nurses with strong clinical assessment skills, a sound understanding of the National Framework for NHS Continuing Healthcare, and a passion for improving outcomes for diverse communities across North West London.

  • Undertake comprehensive assessments to identify healthcare needs and determine eligibility for NHS Continuing Healthcare.
  • Develop, coordinate and review person-centred care plans that promote positive outcomes and wellbeing.
  • Manage a caseload of individuals with complex health needs, ensuring care remains safe, effective and responsive.
  • Act as an advocate for service users and families, ensuring their views and preferences are reflected in care planning.
  • Work collaboratively with multidisciplinary teams, local authorities, care providers and other stakeholders.
  • Lead or contribute to case conferences, MDT meetings and care reviews.
  • Identify, assess and manage risks, implementing safeguarding measures where required.
  • Ensure safeguarding concerns are acted upon in line with local and national policies.
  • Monitor the quality, effectiveness and value of commissioned care packages.
  • Maintain accurate and timely case records, assessments and reports.
  • Liaise with finance and commissioning colleagues regarding care package costs and placements.
  • Contribute to quality improvement initiatives, service development and the implementation of best practice.
  • Provide specialist advice and support to colleagues, service users and partners.
  • Manage competing priorities effectively while working both independently and as part of a team.
  • Support the management of appeals and complex cases in collaboration with the Clinical Lead.
