Care Navigator/ Receptionist (fixed term)
Park Medical Practice - Care Navigator/Receptionist. Part time position (7:45 am 6:30 pm) reporting to the Patient Services Manager.
Overview: The role is to manage front desk and telephone duties, ensuring timely response to patient and staff inquiries, and to signpost patients to the appropriate clinician or service.
Key Responsibilities
- Direct patients appropriately to relevant services.
- Provide excellent written and verbal communication with patients, carers, voluntary, social, primary and secondary care providers.
- Demonstrate strong IT skills and maintain confidentiality.
- Respond appropriately to emergency situations and report incidents that compromise health & safety.
- Signpost patients to the most appropriate service.
- Book appointments, give out test results following practice guidelines, arrange home visits, and process emails/prescription requests.
- Provide general reception duties including greeting and directing patients/visitors, registering new and temporary patients, handing out reports/prescriptions/patient information, preparing and tidying consulting rooms, maintaining health & safety awareness, housekeeping of the waiting area, and chaperoning as requested.
- Input and extract information from the computer system, collect and receipted payments for non NHS services, and open/close the surgery.
- Attend performance & development reviews and training as required.
- Perform rapid health triage and other duties as required.
- Learning and Development
Individual induction, regular reviews, annual appraisal and personal development plans will be provided.
Training needs will be identified and undertaken as part of professional growth.
