The post holder will work as part of the Alliance Rehabilitation Team, an integrated multi-disciplinary team, and will assist in the provision of care co-ordination to individuals with enduring long-term mental health difficulties and complex support needs in Lambeth. The post holder will be a care co-coordinator, as outlined in the Care Programme Approach (CPA) who will be working within a vibrant Multi disciplinary Team (MDT). The post holder will be responsible for assessing, planning, implementing and evaluating plans of care for a defined caseload within a multi-disciplinary team setting. They will be expected to work flexibly, supporting and contributing to the development of the service as a whole.
The Alliance Rehabilitation Team (ART) sits within the IPSA (Integrated Personalised Support Alliance). The IPSA comprises an Alliance contract between SLAM, CCG, Lambeth Social Care, and two voluntary sector organisations, Certitude and Thamesreach. Together IPSA delivers intensive rehabilitation and personalised packages of support to people with complex mental health support needs in Lambeth. It aims to reduce use of rehabilitation inpatient wards and to offer more alternatives to institutional settings, through developing more supported living settings and more independent living opportunities.
Main Duties of the Job
- To act in the clinical role as a Care Co-coordinator for a caseload of clients with enduring long-term mental health problems, on CPA and to actively monitor and support clients on the caseload.
- Undertaking comprehensive assessments of clients’ needs/strengths, including assessments of capacity (including both mental health and social care support needs).
- To undertake and record full assessment of risk.
- To plan, implement and evaluate risk management plans and other protection plans, in conjunction with the client, multi-disciplinary team and other agencies.
- To co-ordinate and record CPA meetings, ensuring that the information is entered on to the ePJS and CPA records are sent out to appropriate parties.
- Ensure that intervention aims at supporting, teaching and guiding individuals towards their optimum level of functioning guided by the recovery model, and principles of personalisation and co-production.
- To ensure effective communication and liaison with ART team members and placement settings to ensure co-ordination of integrated care.

Key Responsibilities
- To act as a Care Co-coordinator for a caseload of clients with enduring long-term mental health problems, on CPA and to actively monitor and support clients on the caseload.
- To undertake comprehensive assessments of clients’ needs/strengths, including assessments of capacity.
- To plan, implement and evaluate care/support plans, risk management plans and other protection plans, in conjunction with the client, multi-disciplinary team and other agencies.
- To undertake and record full assessment of risk.
- To co-ordinate and record CPA meetings.
- To attend all clinical meetings and case conferences as required.
- To ensure that all clients have a documented Care Plan (including a Crisis and Contingency Plan) and are reviewed and evaluated on a regular basis.
- To work in partnership with ART and IPSA colleagues to contribute facilitating the move-on or step-down pathways and transition process.
- To provide Social Circumstances reports for Mental Health Review Tribunals when required.
- To provide cover for colleagues when on annual leave/sick/study leave, and to participate in provision of duty cover.
- To take every reasonable opportunity to improve and maintain professional knowledge and competence.
- To participate in regular supervision and appraisal of own casework with appointed supervisor and team leader.
- To attend all team development workshops and other training opportunities (both statutory and otherwise).
- To provide statistical information in accordance with service demands.
- To contribute to the development of team policies and procedures and ongoing evaluation to ensure the continued development and improvement of the service.
- Report any service deficiencies to Team Leader.
Person Specification
- Registered Mental Nurse with evidence of post-qualification training and current registration.
- Previous experience of developing good working practices with external agencies.
- Evidence of working with clients with enduring mental health problems.
- Experience of working within a multi-disciplinary team.
- A working knowledge of all relevant legislation including the Mental Health Act, Mental Capacity Act and the Care Act.
- Knowledge of Care Programme Approach.
- Understanding of the principles of the recovery model.
- Understanding of and commitment to Equal Opportunities Policy.
- Risk Assessment and management skills.
- Skills in care coordination as outlined in Care Programme Approach and Care planning with client involvement.
- Ability to communicate effectively verbally and in writing, including for the delivery of training.
- Ability to work independently, time management, work under pressure and meet deadlines.
- Post qualification experience in community mental health services.
- Experience in delivering training.
- Experience in working with residential settings.
