Care Coordinator - Clinical Hub Services

The goal of this service is to support patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital.

This role plays a vital part in achieving this.

The Care Coordination Service aims to improve the coordination of a wide range of care provided to specialist palliative care patients and their families within the Royal Trinity catchment area.

The key elements of the role are: To support the single point of access to all Trinitys care services To be the point of contact for all queries from patients; their carers and health and allied health care professionals To ensure patients, their friends and families are allocated to the right service, with the right professional, at the right time.

You will be working as part of a team under the direction of the Lead Coordinator.

However, you will need to use your own initiative to liaise with specialist palliative care patients, their families and healthcare professionals with the key aim of improving access to and coordination of services provided by other health, social and voluntary care agencies.

The nature of the role may involve interactions with distressed and anxious patients and their carers and families.
