The predicted salary is between 37338 - 44962 £ per year.
You will work alongside the Integrated Discharge Team and ward teams to provide support and advice on discharge. This role includes completing referrals to other services, working to tight deadlines, and communicating with patients, families and the wider MDT. Being a point of contact with both the ward team, patients and families is vital to help reduce the length of stay for patients.
Responsibilities include:
- Coordinating the ward’s discharge processes.
- Ensuring patients, families and carers are involved from initial admission to actual discharge.
- Communicating regularly with patients and relatives to promote and maintain understanding of discharge planning and progress.
- Acting as the focal point of contact for communication regarding discharge information and planning for families, patients and the MDT.
- Liaising with other agencies both internal and external to the trust to provide a holistic approach to discharge.
- Ensuring relevant processes are followed to enable timely access to social care, rehab facilities and all community services.
- Early identification of potential problems that may affect discharge and development of initial plans to address those issues, with escalation to relevant teams.
