A fantastic opportunity has arisen to join our well-established Community Nursing teams in Bracknell as a Community Matron. Working as part of an integrated team, you will proactively manage a complex caseload of high-risk patients, using your advanced clinical skills, clinical reasoning and autonomous decision-making to deliver high-quality, personalised care.

You will play a key role in supporting patients with multiple long-term conditions, promoting shared decision-making and proactive self-management to improve health outcomes, quality of life and reduce avoidable hospital admissions. As a senior member of the community nursing team, you will work closely with colleagues across the wider multidisciplinary team and other services to coordinate care and ensure patients receive the right support at the right time.

You will take a lead role in managing patients with complex needs, accessing appropriate services and using your assessment and independent prescribing skills to support effective clinical management. You will also have an important role in supporting and developing junior colleagues, sharing your expertise and contributing to the delivery of high-quality community nursing care.

You will be NMC registered, hold a Non-Medical Prescribing qualification at Level 6/7 and significant community nursing experience. If you are an experienced senior nurse looking to make a real difference to patients in their own communities, we would love to hear from you!

The post holder will work full time within an integrated community nursing team to proactively manage a complex caseload of high-risk patients, applying advanced clinical skills, clinical reasoning and practising autonomously. The post holder will promote shared decision making with individuals regarding their care plan and encourage proactive self-management of long-term conditions with the aim of avoiding crisis resulting in unplanned hospital admission.

The role requires the applicant to take a lead role in managing patients with complex needs to avoid hospital admission and access services as required. The post holder will be a senior nurse supporting community nursing using established independent prescribing and advanced assessment skills, upskilling junior members of staff and supporting scheduled caseloads.

  • Able to use advanced assessment skills and expert knowledge to perform comprehensive assessments of physical, mental health and social care needs that addresses the combined impact of all of the patient's conditions in order to improve health outcomes and quality of life.

  • Able to identify and plan preventative measures and anticipatory care needs.
  • Have a high level of clinical knowledge and clinical reasoning skills.
