The Transitions CYP Support Nurse will be part of the Transitions team to support children and young people (CYP) with orthopaedic, rehabilitation and long-term health needs as they transition from children to adult healthcare services.
Working as part of the Safeguarding and Vulnerabilities Team, the post holder will contribute to care planning, assessment, implementing reasonable adjustments and transition support under the direction of the Transitions CYP Nurse and Named Nurse, ensuring that young people receive person-centred care that promotes independence, wellbeing and positive health outcomes.
The role will have a particular focus on supporting vulnerable young people and those requiring additional support or advocacy support throughout their transition journey.
The post holder will work closely with the Transitions CYP Nurse (band 6) who will oversee daily activity whilst working collaboratively with clinical teams, families, carers, education providers, social care and external agencies to ensure a smooth and effective transition into adult services empowering young people to take control of their health and promoting better long term health outcomes.
Named Nurse for CYA and Transitions (Band 7) will provide strategic and operational oversight of the Transitions service.
- To work as part of the Vulnerabilities Team and support the Transitions Nurse:
- Practise always in accordance with the requirements of the Nursing and Midwifery Council (NMC)
Code: Professional Standards of Practice and Behaviour for Nurses, Midwives and Nursing Associates.
- Maintain active NMC registration and remain professionally accountable for their own practice.
- Work within the scope of their competence, recognising limitations and seeking support or supervision where appropriate.
- Demonstrate a commitment to continuous professional development, maintaining knowledge and skills relevant to transition, safeguarding and vulnerability services.
- Participate in supervision, appraisal, mandatory training and reflective practice activities.
- Contribute to the development of evidence-based practice and support the implementation of national and local policies, procedures and guidelines.
- Maintain professional boundaries and uphold confidentiality in accordance with information governance, data protection legislation and Trust policies.
- Support the Transitions CYP Nurse in clinical audits, quality improvements and service development activities to enhance patient outcomes and experience.
- Contribute to creating an inclusive environment that promotes equality, diversity and inclusion and reduces health inequalities for vulnerable young people
- To undertake any other appropriate role or duty as instructed by line manager

- NCEPOD recommendations relating to transition, continuity of care, multidisciplinary working and the management of vulnerable young people with complex healthcare needs.
- The Care Quality Commission (CQC) expectations regarding person-centred transition planning and safeguarding.
- Relevant safeguarding legislation, including the Children Act 1989 and 2004, Care Act 2014, Mental Capacity Act 2005 and Working Together to Safeguard Children.
