The Royal Orthopaedic Hospital NHS Foundation Trust in Birmingham is recruiting a Transitions CYP Support Nurse to work within the Transitions team, supporting children and young people with orthopaedic, rehabilitation and long-term health needs as they move from paediatric to adult services.
You will work with the Safeguarding and Vulnerabilities Team, contribute to care planning, assessments, reasonable adjustments, and transition support, ensuring person-centred care that promotes.
