An exciting opportunity has arisen for a motivated and enthusiastic Band 6 Occupational Therapist to join our friendly and supportive Intermediate Care Team.
The post is 30 hours per week on a permanent contract.
This peripatetic role offers the opportunity to work across our inpatient intermediate care units, supporting patients on their P2 journey.
We work to the 'Discharge to Assess Model' using the 'Home First' approach, with the emphasis on carrying out essential assessments and OT interventions to maximise independence and to facilitate discharge.
Working from Perry Tree Centre (PTC), you will be part of a dynamic multidisciplinary team committed to delivering high-quality, patient-centred care.
We are looking for a passionate and motivated OT with previous In-Patient experience who is committed to continuing to develop the OT role on our In-Patient wards, taking into account the current and changing climate of the NHS.

As this post involves carrying out regular Access Visits to patient's homes, the successful applicant will ideally need to be a car driver.
Main Duties of the Job
You will
- Provide timely occupational therapy assessment and intervention for patients within intermediate care inpatient settings.
- Work across P2 rehabilitation units, supporting safe and timely discharges.
- Undertake holistic assessments to maximise independence, functional ability, and quality of life.
- Complete discharge planning, equipment provision, and environmental assessments to facilitate smooth transitions of care.
- Work collaboratively within a multidisciplinary team including therapists, nurses, medical colleagues, and support staff.
- Support patients, carers, and families to achieve personalised discharge goals.
- Supervise, teach, and support junior staff, therapy assistants, and students where appropriate.
- Participate in multidisciplinary meetings, case conferences, and discharge planning discussions.
- Actively contribute to service development initiatives, quality improvement projects, and innovation within Intermediate Care Services to enhance patient outcomes and service efficiency.
- Identify opportunities for improvement and support the implementation of evidence-based practice, new pathways, and service redesign projects.

- To recommend & prescribe necessary equipment to improve patient's safety and independence and to instruct carers, staff and relatives how to fit, use and maintain prescribed equipment safely.
