We are looking for Personalisation Support Workers who are passionate about person-centred care and active support. In this role, you’ll help individuals with complex needs unlock their potential through meaningful activities, positive relationships, and high-quality support. As a Personalisation Support Worker, you’ll play a central role in delivering personalised, engaging, and inclusive support. You’ll help design and lead activities that reflect each individual’s interests, abilities, and goals, supporting wellbeing, independence, and choice every day.
Key Responsibilities
- Plan, coordinate, and deliver accessible, person-centred activities.
- Support individuals to take part in meaningful experiences and develop life skills.
- Lead activities within the Hub and wider service, role modelling active support.
- Adapt activities using assistive technology and total communication methods (e.g. Makaton, PECS).
- Encourage community access and maximise independence.
- Support colleagues, volunteers, and agency staff to ensure inclusive participation.
High-Quality Person-Centred Support
- Provide compassionate care that promotes dignity, choice, and wellbeing.
- Support individuals to communicate preferences, make decisions, and self-advocate.
- Deliver personal care and health support in line with individual care plans.
- Encourage participation in daily life within the Day Service.
- Use safe manual handling techniques and support mobility needs.
- Uphold safeguarding responsibilities in line with policy.
Quality, Recording & Teamwork
- Maintain accurate, up-to-date person-centred support plans.
- Record care and support using organisational systems.
- Lead by example in care practice and documentation.
- Work collaboratively with Senior Support Workers, Hub Leaders, families, and professionals.
- Contribute to training, supervision, and continuous professional development.
