An exciting opportunity has arisen for an experienced Mental Health Clinician within the CHC/CC/Complex Care team within Cheshire & Wirral Partnership NHS Foundation Trust. The successful applicant will have a working knowledge of 117 aftercare legalities and planning, Mental Capacity Act, Safeguarding, Deprivation of Liberty Safeguards, experience of undertaking assessments, case and risk management and personalised care agenda will be essential for this role.
The post-holder will undertake various functions as part of the role which will include:
- Undertaking health needs assessments
- Facilitation of multi-disciplinary assessments and reviews
- Supporting individuals with case management
- Providing information on legal matters which might include COPDOLs, welfare applications as well as S21a cases
- Staff leadership and management as necessary
We require a motivated and experienced Registered Mental Health Nurse (or appropriate alternative professional registration) who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to commission safe, appropriate care and support the wider system with patient flow.
The successful candidate will be joining an established team and will receive role specific induction following the CWP trust induction. This is a specialist role which includes completing assessments and reviews and making commissioning arrangements (including funding applications). Attending MDT and professionals’ meetings as well as meetings with individuals and their representatives and care providers. Supporting wider ICB colleagues and working with Local Authority partners and attending MADE events to support patient flow. Working on complex cases where there may be legal input or challenge.
The post holder will be a key member of the team and will be expected to work autonomously, ensuring effective management and assessment of clients, meeting individual ongoing health care needs for mental health clients, in conjunction with the Continuing Health Care Team and ICB and coordinating the process of securing health funding.
