Key Responsibilities
- Care Package Brokerage and Coordination
- Arrange and record care packages accurately using designated case management systems, ensuring compliance with organisational procedures and quality standards.
- Source and broker care and support services from contracted providers in line with assessed needs, care plans, and available resources.
- Manage daily referrals and demand for care services, prioritising urgent cases including hospital discharges and safeguarding situations.
- Support the coordination of out-of-area placements, ensuring continuity of care and adherence to relevant policies and procedures.
- Ensure care arrangements are implemented within required timescales to support effective service delivery.
