We are delighted to announce an opportunity for a Rehabilitation Support Worker to join our Intermediate Care Rehabilitation team at the Bexhill Irvine Unit. In this role, you will play a vital part in supporting rehabilitation patients by implementing their personalised care plans and managing rehabilitation dosage to prevent deconditioning and promote positive outcomes.
A flexible and adaptable approach is essential, ensuring seamless collaboration across therapy and nursing teams to meet service demands. The role of the Rehabilitation Support Worker is integral to achieving these goals:
- Conduct rehabilitation activities, including group sessions and individual patient rehabilitation, as directed by the rehabilitation team, ensuring regular and accurate progress updates are provided to therapy leads.
- Collaborate with patients to establish clear rehabilitation goals and work alongside the wider rehabilitation team to develop and implement a comprehensive, personalised rehabilitation plan.
- Work flexibly with the broader intermediate care teams including therapy and nursing teams to ensure patients receive safe, effective, and high-quality care and treatment.
- Support the professional development of colleagues across nursing and therapy teams by sharing knowledge and skills to optimise the collective expertise for patient care.
- Work with broader intermediate care teams during staff shortages to ensure patients consistently receive safe and effective rehabilitation.
