More details about the role We need a highly motivated, enthusiastic individual to join our dynamic and proactive team.

Applicants will require good assessment, clinical and communication skills in order to deliver evidence-based nursing care to patients within their home environment, recognising when a patient is deteriorating and intervene and elevate concerns appropriately to optimise quality of life and reduce the risk of unnecessary hospital attendance.

The successful candidate will be able to demonstrate the ability to provide a high standard of care and have good communication skills.

Applicants must be well organised with a professional and approachable manner.

They must be prepared to work as part of a team and autonomously.

A full driving licence and a mode of transport suitable to cover a large geographical area will be essential.
