Integrated Neighbourhood Team Care Coordinator
Experience Ideally you will have experience of delivering healthcare and have good organisational and administrative skills.
Job Responsibilities Clinical Skills Excellent communication skills to review patients on the phone or face to face.
To be able to identify sick patients and seek advice appropriately.
Complete clinical tasks such as, phlebotomy, processing of samples of e. g. urine, ECGs, weight, blood pressure monitoring and simple dressings.
Refer to trained staff when appropriate.
Undertake routine tasks related to individuals health and wellbeing.
Assist in the delivery of care to meet individuals health and wellbeing needs.
Establish and maintain communication with people on routine care matters.
Assist in maintaining own and others health and safety and security.
Acknowledge the risk of Healthcare Associated Infections (HCAI) and understand own responsibility as agreed with line manager in the prevention and control of HCAI.
To maintain standards of infection control and ensure correct infection control procedures are followed when cleaning equipment.
To meet BCHC values, whilst delivering care to patients and relatives.
Carry out any other duty that reasonably falls within the general nature and level of responsibility of the post.
Ensure all concerns are escalated in line with best practice.
Endure confidentiality is maintained.
Administrative Skills Accurate daily tracking and reporting of patient updates, notes, results and next step pathway plans.
Enter accurate and maintain real time data in clinical notes systems.
Facilitate and document minutes of MDTs.
Provide backup holiday cover to Administrative Senior Care Coordinator.
Tools Proficient in MS excel spreadsheets.

Governance Ensure own actions help to maintain quality.
Ensure own actions support equality, diversity and rights.
Provide and receive patient information, ensuring that all patient information is provided, received and recorded in accordance with the Data Protection Act and PML Policies.
To ensure that Clinical Governance and risk management are embedded within daily practice.
Responsible for stock taking, ensuring adequate supply and correct storage.
Ensure all patient information is accessible and up to date.
Assist with patient referrals and allocation of work.
To attend team meetings when requested whether virtual or face to face.
