The opportunity has arisen to be part of our expanding multi-disciplinary frailty team and deliver care in our Proactive Frailty Care Planning Clinics across the six Integrated Neighbour Team locations in Buckinghamshire.

In our Proactive Frailty Care Planning clinics we conduct Comprehensive Geriatric Assessments (CGAs) on moderately frail patients to identify and solve problems before things go wrong. The post holder would work as part of an MDT within the clinics but would autonomously assess the therapy needs of the patients and provide the necessary solutions to ensure patients can lead healthier, more independent lives.

The Physiotherapist will provide expert care and treatment to patients within agreed protocols and clinical guidelines, working as an autonomous practitioner within their own competence; undertaking frailty health assessments and providing appropriate advice on therapy requirements.
