We are seeking a compassionate and dedicated ASW - Social Worker to join our Hospital Pathway and Reablement team at Grade 8 in the Social Care Qualification stream. This vital role involves working collaboratively within a multidisciplinary team to support patients transitioning from hospital to home or alternative care settings, ensuring they receive the appropriate level of care and support to maximize their independence and well-being.

As an integral member of the social care team, you will play a key role in assessing the complex needs of individuals, developing tailored care plans, and coordinating interventions that promote rehabilitation and reduce hospital readmissions. This position requires a proactive, empathetic, and resourceful professional who is committed to delivering high-quality person-centered care.

You will work closely with healthcare professionals, families, and community services to facilitate smooth and effective pathways that enable recovery and reablement outcomes.
