The Heart Failure Specialist Nurse role at Tameside and Glossop Integrated Care NHS Foundation Trust is a fixed-term maternity leave post.
The ideal candidate will join our established Heart Failure Service and contribute across acute, community and outpatient settings.
You will manage a specialist caseload, perform autonomous clinical assessments, lead nurse-led clinics and optimise guideline-directed medical therapy, working with the MDT to deliver patient-centred care and support families.
