An exciting opportunity has arisen within Intermediate Tier Services, rated Outstanding by the CQC, for an Advanced Clinical Practitioner to join the multidisciplinary team at the Stamford Unit.
The Stamford Unit is a 96-bed Intermediate Care facility providing rehabilitation, reablement, complex discharge support, step-up admissions and end-of-life care.
Working alongside GPs, Therapists, Nurses and Social Workers, you will play a key role in delivering high-quality, person-centred care to patients with complex health needs.
This hospital-based community role combines autonomous clinical practice with multidisciplinary working.
You will undertake advanced assessment, diagnosis and management, lead care planning and support interventions that promote recovery, independence and safe discharge.
Working at the interface of hospital and community care, you will contribute to innovative models that support admission avoidance, timely discharge and care closer to home.
We are seeking an experienced and motivated clinician with a passion for improving outcomes for older people.

- Work across the four pillars of advanced practice, demonstrating expert clinical judgement, autonomous decision-making and leadership in the assessment, diagnosis and management of patients with complex health needs.
- Provide advanced clinical care for patients requiring rehabilitation, reablement, complex discharge planning, end-of-life care and ongoing medical management.
- Undertake comprehensive assessments, develop management plans, request and interpret investigations, prescribe where qualified and coordinate care to optimise patient outcomes.
- Work collaboratively with GPs, Therapists, Nurses, Social Workers and specialist teams to deliver safe, effective and person-centred care.
- Contribute to multidisciplinary team discussions, treatment escalation planning and discharge decision-making.
- Support service development through quality improvement, audit, evidence-based practice, education and mentorship.
- Contribute to pathway development and promote a culture of continuous improvement and clinical excellence.
- Act as an advocate for integrated working across primary, community and secondary care services to improve patient experience and outcomes.
