An exciting opportunity has arisen for a Band 8a Advanced Practitioner Physiotherapist to lead our specialist Spasticity and Botulinum Toxin Physiotherapy Service. This advanced practice clinical role involves expert assessment, diagnosis and treatment of patients with complex spasticity, including intramuscular botulinum toxin administration, alongside service development, clinical supervision, policy leadership, teaching, audit, and research across Trust sites.

Based at York Hospital within the AHP Neurology service, the post-holder will work autonomously and collaboratively within the MDT to deliver high-quality, evidence-based spasticity management, including botulinum toxin administration. The post-holder will act as the clinical lead for physiotherapy spasticity management across Trust sites, primarily working in adult services but where relevant also to children.

Providing expert assessment, treatment planning and interventions for patients with complex neurological presentations. The role includes delivering specialist care across a range of settings, including outpatients, wards, community settings and patients’ homes, while supporting holistic management and informed patient decision‑making.

The successful candidate will play a key role in teaching, supervision, audit, service evaluation and research. You will support workforce development, deliver training, and contribute to service improvement and innovation within neurology and stroke services.

To provide specialist clinical care for patients with complex, long‑term and often distressing symptoms, focusing on spasticity management and Botulinum Toxin treatment. Deliver holistic care, including physiotherapy assessment, treatment planning, lifestyle advice and support for informed decision‑making.

Lead the physiotherapy contribution to the multidisciplinary spasticity and Botulinum Toxin service, working closely with consultants, physiotherapy injectors, orthotics and wider MDT colleagues. Take a lead role in the Multiple Sclerosis Sativex clinic.

Undertake assessment of highly complex presentations, using advanced clinical reasoning and specialist knowledge to establish diagnoses and deliver evidence‑based treatment plans as an autonomous practitioner.

Lead the MDT Botulinum Toxin service, including intramuscular injection, service development, policy implementation and maintenance of clinical standards. Provide specialist physiotherapy interventions for neurological patients, developing individualised treatment programmes to optimise outcomes and independence.

Lead strategic planning and development of the physiotherapy spasticity service across Trust sites, identifying workforce, training and equipment requirements in line with national guidance, evidence and service demand. Support and lead research, service improvement and innovation across stroke and neurology physiotherapy services.
