Advanced Nurse Practitioner (Primary Care)

Main responsibilities include:

• Clinical Leads and manages the Enhanced Health in Care Homes programme.
• Undertakes weekly ward rounds.
• Supports person-centred holistic assessments of all patients admitted/readmitted to a care home within 7 days.
• Ensures that all residents have a personalised support care plan within 7 days of re/admission.
• Develops and manages highly specialised programmes of care and care packages.
• Provides highly specialised advice concerning care beyond the scope of normal advanced clinical practice.
• Makes fast track referrals.
• Works autonomously and is accountable for professional actions.
• Undertakes clinical nursing practice at an advanced level using expert knowledge and clinical skills.
• Assesses patients with a range of acute, non-acute, and chronic medical conditions.
• Takes history, conducts appropriate physical examinations, formulates differential diagnoses, and carries through management treatment plans ensuring relevant follow-up.
• Manages undifferentiated undiagnosed conditions and identifies red flags and underlying serious pathology.
• Uses complex decision-making to inform diagnosis and complete management of episodes of care.
• Actively takes a personalised care approach and population-centred care approach.
• Makes referrals to other healthcare professionals in primary and secondary care where appropriate.
• Prescribes medications as appropriate.
• Maintains accurate record keeping in Emis Web.
• Makes critical judgements to satisfy job expectations.
• Advises and supports others where standard protocols do not apply.
• Visits patients at home and within care homes as part of the wider clinical team.
• Communicates complex and sensitive information effectively to patients/carers.
• Fosters good public relations by ensuring courtesy and discretion when dealing with patients and their relatives.
• Supports and advises patients on health promotion and self-management.
• Recognises situations detrimental to health and acts on findings.
• Ensures care given is supported by the best available evidence and local policies.
• Maintains professional competency and is current for NMC revalidation.

Education:

• Completes relevant training to provide multi-professional clinical practice and CPD for other roles within Primary Care.
• Links with the Training Hub and colleagues across BNSSG to ensure Care Home staff are aware of local training opportunities.
• Organises at least one training session per year for PCN and Practice staff on issues relating to the care of frail elderly people.

Research:

• Continuous involvement in research/evaluation and/or development work.
• Actively contributes to PCN/Service/Team audit/Research activity as required.
• Instigates audit activity for new and existing services.
• Identifies, applies, and disseminates research findings relating to clinical practice.

Leadership:

• Establishes strong working relationships with nursing and management staff in care homes.
• Works with the GP lead for care homes to identify and follow up opportunities for improvement of care.
• Provides clinical support and advice to colleagues including Care Co-ordinators and Community Paramedic.
