HCRG Care Group is seeking an Advanced Clinical Practitioner to join the Hospital at Home team supporting Bristol, Swindon, and North Wiltshire localities.

You will deliver acute, autonomous care to patients at home, bridging hospital and community services.

You will assess, plan and implement treatment, supervise juniors, and work within four pillars of Advanced Practice, maintaining NHS standards and patient safety.

Travel between localities is required.
