We are seeking a skilled, compassionate and motivated Paramedic to provide advanced clinical assessment, treatment and support across COCO PCN. The role combines home visits and practice-based clinics, assessing, diagnosing, treating and appropriately referring patients to provide safe, effective and timely care. Working alongside GPs and the wider multidisciplinary team, you will support urgent and routine care, improve access to primary care, reduce avoidable hospital admissions and improve patient outcomes. You will be expected to demonstrate strong clinical decision-making, work autonomously within your scope of practice, contribute to service development and deliver evidence-based, patient-centred care.

Key Responsibilities

  • Provide assessment, diagnosis, treatment and follow-up for patients in their homes and GP practices.
  • Take medical histories, perform examinations and arrange appropriate investigations and treatment.
  • Develop and review care plans, escalating or referring patients where appropriate.
  • Recognise and respond appropriately to deteriorating or acutely unwell patients.
  • Provide health education and support patients and families with self-management and prevention.
  • Work collaboratively with GPs, nurses, pharmacists, social care and the wider multidisciplinary team.
  • Ensure clinical equipment is maintained and fit for use.

  • Maintain strict confidentiality and comply with information governance, data protection and organisational policies.

  • Participate in appraisal, clinical audit and quality improvement where appropriate.

  • Complete mandatory training and maintain relevant knowledge and competencies.

Person Specification

  • Essential HCPC-registered Paramedic.
  • Advanced Life Support (ALS) qualification.
  • Level 7 postgraduate qualification or equivalent advanced clinical experience.
  • Strong clinical assessment, diagnostic and decision-making skills.
  • Ability to work autonomously within scope of practice.
  • Good understanding of UK healthcare regulations and professional standards.
  • Strong communication, organisational and problem-solving skills.
  • Compassionate, patient-centred approach with a commitment to quality care.
  • Ability to remain calm and professional under pressure.
  • Commitment to multidisciplinary and collaborative working.
  • Full UK driving licence and access to a roadworthy vehicle.
