Allied Health South Lincolnshire Limited is seeking a Registered Nurse to join our Specialist Neighbourhood Nurses team based at Long Sutton Medical Centre. The 0.6 WTE post (3 days per week) is permanent and offers pro rata salary.
The role supports The South Lincolnshire Rural PCN and GP Practices, focusing on reducing unplanned hospital admissions and community care. You will work autonomously within a multidisciplinary team, handling home visits, care planning, and liaison with GPs.
